All weapons systems malfunction occasionally. Even precision engineered Israeli ones.

The projectiles typically fired from Gaza into Israel have a very high-rate of malfunction. The "Raymond Reddington" character in the US TV show; "The Blacklist" actually gave a pretty accurate description of them;

"They just poorly welded water pipes. Packed with A.N.F.O explosives."

There hasn't been an actual war in Gaza since 1973. However if I had the time I could dig you up some statistics on how many projectiles fired from Gaza have malfunctioned, landing in Gaza, in each of the last exchanges of fire. Off the top of my head I think it's typically 15-20%.

The only thing which is causing me any doubt. Is that it happened almost exactly to the minute that Ukrainian Nazis fired a missile at Polish Nazis in the hope of unleashing a Nuclear Holocaust against Russia. During the COP 27 Summit.
